We are interested community members who want to support people receiving palliative and end of life care in our community; either at home, in hospital or hospice.

To educate people about palliative and end of life care as well as all things pertaining to living while dying.

To build an active volunteer base to support patients and their families.

Referrals

North Zone Continuing Care Access

Physician referral not required. Families and patients can self-refer by calling 1-855-371-4122.

Continuing Care Access (CCA) has a toll free number staffed by Registered Nurses who will discuss your health care needs, complete an assessment and send a referral to the most appropriate AHS health care provider or service. The Registered Nurse may also provide callers with resources and supports available in their community.

Palliative Care vs.

End of Life/Hospice

Palliative Care

It is normal to be concerned when one hears the term 'Palliative Care'. It is often confused with imminent death or end-of-life/hospice care. Palliative Care is also sometimes referred to as comfort care or supportive care.

Palliative Care aims to improve the quality of life of patients and their families facing the problems associated with living with a life-limiting illness or serious health condition, through the prevention and relief of distressing symptoms.

Palliative Care can be provided at any time following diagnosis with a serious illness. When Palliative Care is provided early in the course of disease, it offers extra support while medical treatments continue. The team can include physician, nurse practitioners, palliative care resource nurses, social workers, occupational therapists, recreational therapists, spiritual health practitioners, mental health therapists, respiratory therapists and clinical nurse educators.

End of Life Care/Hospice

Is care provided to an individual and their family when they are approaching a period of time closer to death. It is usually within their last weeks to days of life when care needs and services may need to be increased.

Where the Rivers Meet

Sagitawah Hospice Committee recognizes that we are situated on the traditional territory of Treaty 6 and Metis District 8. We honour those who since the beginning of time have cared for and supported their family, friends, and community through the cycles of life.
